In preparation for a comprehensive evaluation with Dr. Stivi, you are asked to complete the Patient Information Packet. Please give yourself ample time to complete it accurately so that Dr. Stivi will have the necessary information to begin your journey to optimal health. You may elect to fax, mail, and/or bring your completed information packet to the New Health Institute on the day of your appointment.
